Team Registration
For the U8-12 Season 2009
(activated on 11/8/08 before the U8-12 Coach Meeting on 11/12/08)
To register a team for the U8-12 Season 2009 two steps are required. First, the team registration information must be submitted. Second, the team registration fee must be paid.
Step One:

1)

complete all the information fields in the Team Registration Form; if the information is not applicable to your team use "na" for not applicable.

2)
please be sure that all information is entered correctly without spelling errors (especially the email addresses) and that all contact numbers are correct and current (note: if you only enter one contact and it is incorrect we will not be able to contact you).
3)
 print a copy of the Team Registration Form for your records before you submit the form.
4)
 after you submit the Team Registration Form you will then go to Step 2 for payment.
5)
 the registration of a team requires a separate Team Registration Form and a separate payment for each team.
6) Travel Schedule Request Form: if your team will be traveling to tournaments or competitive events during the OL competitive season you must submit the Travel Schedule Request Form
Step Two:
1)
 select one of the the two payment options.
2)
 pay the team registration fee by 1) credit card or by 2) internet check.
(note: personal checks are not acceptable for team registration fees; if you submit a regular check with the Team Registration form it will not be processed; you must pay by credit card or by internet check through the OL Store).
4)
the team registration fee for one team in the U8 Season is $400 and for the U9-12 Season the team fee is $500.
the on-line payment submittal deadline is 4:00 pm Friday, November 21, 2008.
on-line credit card or internet check payment for the team registration will be processed immediately when you "check-out" in the Oahu League Store; all payments for team registrations are non-refundable.
