		Use this form to request a review of a team's placement in the competitive format.
		In order to be considered for a change in your competitive placement you must present documentation that the team should be placed differently than as listed in the previous seasonal team standings and team list (check standings)
		Provide this documentation in the form below.
		After you submit the Competition Form, print a copy of the "Competition Form Submittal Page" for your records.
		After submittal you will receive an email confirmation that the "Competition Form Submittal Form" has been received; if you do not receive an email confirmation receipt in 48 hours, send an email to schedule@oahuleague.com listing the information required for the form.
